Care Management
The Care Management Program supports individuals in their desire to continue living in their own homes or in other settings with maximum independence and dignity.
Professional Certified Care Managers work with individuals and families to support healthy living, maximize safety, and improve quality of life. This intensive 24/7 service provides guidance, resources, respite, and peace of mind to family caregivers, whether near or far-away.
ASSESSMENT
Care Management begins with a thorough individualized assessment which focuses on current physical, emotional, mental, and cognitive health as well as safety within the current environment. This assessment becomes the basis for a set of recommendations and a personalized plan of care.
CARE PLAN
Based upon the comprehensive assessment, the Care Manager, in conjunction with the individual and family, develops a detailed plan of action. This plan outlines options, recommendations, strategies and services for maximizing independence.
ON-GOING CARE MANAGEMENT
Once an Assessment has been completed and a Care Plan has been developed, you may choose ongoing care management services. Care Managers will use their expertise in elder care and navigating the health care system to implement the plan of care, and to ensure that all appropriate services are in place, are coordinated, and are contributing fully to the well-being of the individual. The Care Manager can arrange and coordinate services such as:
- – home health and personal care
- – physical and occupational therapies
- – bill-paying
- – medication management
- – companionship to reduce isolation
- – podiatry
- – home adaptations for safety and comfort
- – in-home medical care
- – selection of assisted-living or other care environments

HEALTH ADVOCACY
Care Managers advocate with primary care physicians, specialists, and all members of the health care team to ensure that medical care is coordinated. Care Managers can accompany individuals to medical appointments; review and recommend improvements in care for those living in assisted-living and nursing facilities; advocate for individuals during crises such as hospitalization.
GERIATRIC SPECIALIZATION
JCS Care Managers are Licensed Nurses and Social Workers who have expertise in Aging. Professional training and certification ensure the knowledge and skills necessary to address the physical, cognitive, emotional, and medical needs of elders.
